Your Personalized Care Team

Our mission is to care for each person the way he or she wishes to be cared for. The Pacific Hospice Care team includes professionals, paraprofessionals and volunteers who are sensitive and responsive to individual patient and family needs:

  • Nurses are trained caregivers who provide hands-on care, education and support about the patient's illness and what to expect. An on-call nursing team is always available after hours and on weekends for visits or phone consultation. Other specialists, such as nutritionists and physical, speech or occupational therapists, may be added to a patient’s team as needed.
  • Social workers are available as friendly and skilled listeners to provide emotional support and help with problem solving, financial issues and accessing various community agencies for additional support programs.
  • Physicians work with the patient’s physician as well as to make house calls as needed.
  • Home health aides are available to help with personal care, such as bathing or dental and mouth care, and can help with light cleaning, housekeeping, light laundry, heating meals and short errands.
  • Chaplains offer spiritual support, working with the family’s clergy when available, to address the spiritual concerns of patients and family members within each patient’s individual belief system, as well as addressing concerns of a more generalized spiritual nature;
  • Volunteers are specially trained and an important element of the hospice team, available to spend time when they are needed most.
